Headache’s Origins

Your Chiropractor Plano TX found at AK Chiropractic. Chiropractor Plano by Dr. Gonzalez.

Headaches have many origins, you can think of these as triggers. These may include environmental factors (noises, stress), foods or behaviors (unable to sleep, exercising too much!, blood sugar changes). A small five percent of headaches are simply warning signs caused by physical problems.

95% of headaches are called primary headaches, these include tension, migraine, or cluster headaches. These headaches are not caused by disease. The headache itself is the primary concern in these instances.

A lot of primary headaches are simply associated with muscle tension in the neck. Americans in general are more sedentary than they used to be, and more time is being spent in one fixed position or posture. This causes joint irritation and muscle tension – affecting areas in the neck, back and top of the head, causing your head to hurt.

Here are some things you can do to alleviate these pains:

  • If you spend a lot of time in just one or two fixed positions, like in front of an office computer, just take a break and stretch every hour. The stretches should take your head / neck through a good range of motion.

  • Exercise will also help relieve the pain associated with primary headaches. But if you are likely to get dull, throbbing headaches, avoid too much exercise. Do small activities like walking and low-impact aerobics.
  • Don’t clench your teeth. The upper teeth should never touch the lowers, except when you’re swallowing. This results in stress in the two joints that connect your jaw to your skull – leading to irritation and tension headaches.
  • Drink 8-ounce glasses of water a day to help avoid dehydration – as this leads to headaches.

A migraine is a severe, painful headache that is often preceded by sensory warning signs like flashes of light, blind spots, tingling in the arms and legs, nausea, vomiting, and increased sensitivity to light and sound. The excruciating pain that migraines bring can last for hours or a week.  Here are top five causes of migraines.

1) PAINFUL PERFUMES

For migraine sufferers, any cologne or perfume can be a ticket to a day spent in bed. A 2011 Cephalalgia study found that patients say perfumes and other strong odors triggered attacks.

2) HUNGER GAMES

Many people say that hunger causes their migraines. A 2005 Cephalalgia study found that hunger was tied to migraines, especially migraines with aura, or flashing lights. By contrast, going without food didn’t always cause the everyday tension headaches that most people have occasionally.

3) WINE AND CHEESE

A romantic dinner with wine, chocolate and cheese can be dangerous for a migraine sufferer. Many patients reported that some foods trigger splitting headaches. But while food-based triggers are common – with red wine, chocolate and cheese being the most frequent culprits – experts have not yet figured out which chemicals are actually responsible, according to a 2012 study in the journal Neurological Sciences.

4) WEIRD AIR PRESSURE

Lightning isn’t the only weather-related thing that causes throbbing headaches. Studies have shown that rapid changes in barometric pressure can change the charge concentration in the air and then flood the brain with serotonin, Freitag said. That serotonin release causes head pain.

5) STRESS

Among the most commonly cited triggers, being stressed can spur the flashing lights and head pain of a migraine, several studies suggest. In a 2012 study in the Journal of Health Psychology, personal stress was listed as the one most common cause of migraine pain.

Chiropractic Care Helps Us to Manage Stress

Your Chiropractor Plano TX found at AK Chiropractic. Chiropractor Plano by Dr. Gonzalez.

Your chiropractor uses various techniques and methods to restore proper spinal alignment. The direct result is improved functioning of your nerve system, which directly improves the functioning of all your other body systems.

One of the structural problems addressed by chiropractic care is a “facilitated segment.” This is a section of your spinal cord that is being bombarded by too many nerve signals. These nerve signals arrive at the wrong times and in too great a quantity. Physical pain and other diseases and disorders can develop as a result of a facilitated segment.

Stress makes facilitated segments (and their effects) worse. By aligning your spine, your chiropractor is helping to reduce and resolve these irritated segments of your spinal cord. As a result, the stresses in your life have less physiological impact. You become better able to interact with people and situations, and become better able to manage stress.

We live in stressful times. The economy is tough, global conflicts rage, severe weather events are affecting people in every corner of the globe, and our numerous technological devices don’t seem to be making things any easier. Of course, this is nothing new. Every generation thinks theirs is the best of times and the worst of times. But the result is that people everywhere have high levels of stress.

Sometimes stress is a good thing. Mechanical stress, such as exercise, causes your muscles and bones to become stronger and your nerve system to become smarter. Taking on a new assignment at work or taking a challenging class in school may be stressful, but the effort involved in achieving a successful result will cause you to grow and develop in ways that you might not have imagined. After all, the great philosopher Friedrich Nietzsche famously said “what doesn’t kill you makes you stronger.”

But chronic, ongoing physical and mental stress, the kind that affects us every day, is not good for us. Chronic stress causes real psychological and physical problems. People undergoing chronic stress may develop anxiety, which if not managed effectively may lead to depression. Chronic stress may lead to a variety of disorders and diseases, including arthritis, inflammatory conditions such as Crohn’s disease, gastrointestinal problems such as peptic ulcer, diabetes, high blood pressure, and even heart attacks and stroke. As we are all subject to numerous stresses every day, both personal and work-related, it’s very important for us to develop strategies that will be successful in helping us manage ongoing stresses.

The most important method for managing stress is to attempt to focus your point-of-view, your frame of reference, on the present moment. Our minds are constantly in motion, constantly at work creating new thoughts and new scenarios regarding some potentially stressful situation, or rehashing old conversations, old conflicts, and old problems. The result is that we’re almost never at peace. When we try to sit down and relax for a bit, it’s never too long before our minds start bringing up the exact things we’d like to be able to forget or ignore, at least for a little while. Most of us can’t flip a metaphorical switch and shut off our incessant stream of mostly negative self-talk. But we can learn to remind ourselves to return to the present, to come back to this moment, the one that’s happening now.

The best way to do this is to ask yourself, “is this [what I’m thinking about] happening right now?” If you can see that what’s happening now is that you’re sitting in a chair reading the newspaper, you have a good chance of being able to let go of what your mind is talking to you about. Say to yourself, “I’m right here, right now, and none of that other stuff is actually happening right now.” This will help you create some distance from your self-talk, and the self-talk may even recede into the background for a while. The feeling of calm you might then experience is one you can build on. The more you practice returning to the moment, the more power you gain in being able to manage the stress in your life.

Time magazine just ran the below article (1/24/2013), “Migraine Triggers May Not Be So Potent After All”.

Bright lights and too much exercise are well-known migraine launchers, but the latest study hints that sufferers may not be as sensitive to these triggers as previously thought. Researchers report in the journal Neurology that commonly suspected migraine triggers might not be responsible for a hurting head after all.

Unlike previous studies of migraine triggers, in which sufferers were asked about what conditions or situations preceded a headache, the scientists exposed 27 migraine patients in a lab to flashing lights, intense exercise or a combination of both to provoke a migraine with aura, a type of headache accompanied by often debilitating visual disturbances. Only a handful of subjects experienced any sort of migraine, and those who did had exercised, suggesting that bright lights may not be to blame. The participants ran or used an exercise bike at maximum effort for an hour, while researchers used a combination of lamps, flashes and other visual stimuli to mimic light disturbances for up to 40 minutes in order to study the combined effect of light and exercise. After these sessions, only 11% of the participants — three patients — experienced migraines with auras, and an additional 11% experienced migraines without auras.

“What have generally been reported as sure triggers for migraines are not so sure when you actually expose people to them” says Dr. Jes Olesen, the study’s corresponding author from the University of Copenhagen in Denmark and a fellow of the American Academy of Neurology.

Dr. Stephen D. Silberstein, a professor of neurology at Thomas Jefferson University and the director of the Jefferson Headache Center who co-wrote an accompanying editorial for the study, suggests that some of what people think are triggers may actually be symptoms of migraines instead.

“You eat chocolate and you get a headache. Does that mean chocolate triggers the headache?” Silberstein asks. “What probably happens is the first symptom of your migraine attack is the desire to eat chocolate. Just like when you’re pregnant, you might want pickles or ice cream. That’s one end of the spectrum, where the desire to do something is part of the migraine attack, not the trigger.”

Distinguishing between triggers and symptoms is challenging, not just for those who study migraines but for patients as well. Silberstein says there are some known triggers, such as certain odors, hunger, chemicals in alcohol and hormonal changes linked to menstruation, but that other factors may fall somewhere between an actual trigger and a symptom. How can patients tell? “Everybody with a migraine should try to find out what is triggering their attacks,” Olesen says. “When they have a suspicion, it would be a good idea to try and see if it induces an attack. In most cases, it’s probably not going to be true.”

Both Olesen and Silberstein say there are a number of factors that determine whether these suspected triggers will actually lead to an attack. Patients likely have individual thresholds that vary from day to day and from environment to environment: some days your brain is less vulnerable to certain triggers, while on other days the conditions might be right for a migraine.

Stress, for example, is one of the most commonly reported migraine triggers, and bright lights and exercise may simply be markers for when stress levels are high. The fact that only exercise appeared to trigger migraines came as a surprise to Olesen, considering a majority of the study’s participants believed bright lights induced migraines more than half the time upon exposure. “The patients were almost apologetic to us because they had indicated it would cause an attack, but it didn’t,” Olesen says.

If these suspected triggers aren’t causing migraines, though, why have they been fingered as responsible for the headaches? Most of the evidence linking the triggers to migraines comes from studies in which patients self-reported what they thought were the factors responsible for their headaches; for the most part, these factors weren’t tested in the way that Olesen analyzed the effect of bright lights and exercise on migraines in the lab.

People’s beliefs about migraines may also complicate how researchers explore migraine causes. Similar to the placebo effect, how patients think about migraines will often influence how they experience them. If you’re convinced a certain food will a trigger a migraine, your suspicion might become a reality; if you’re worried you’ll get a migraine, you might just bring about one.

That’s not to say lights will never cause a migraine. Because the study was conducted in a lab setting, the exposure to triggers may not accurately replicate the conditions that launch a real-life headache. For example, if a migraine strikes after you exit a dark movie theater and walk into bright light, it could be the sudden contrast from the darkness of the theater that made the light problematic. Or, you might have been hungry or sleep-deprived before being exposed to a bright light, which would still make the light a trigger, albeit one of a combination of factors that prime your brain for the headache.

Olesen says researchers have a better understanding of migraines without auras — they have identified certain chemical triggers that have led to the development of a new class of migraine drugs, for example — but migraines with auras continue to be more of a mystery. The findings hint, however, that some long-held truths about avoiding triggers that can induce migraines with aura may be outdated.

“Trigger avoidance is overdone,” Silberstein says. “If you know something is going to cause your headache, whether you believe it as a result of conditioning or reality, avoid it. But people who take away everything in life that they love because it might trigger their headache, that’s useless.” He argues that it’s worth trying to find out what truly triggers your migraines, even if the process is painful. That suffering may be worth it in the long run.

Scientists do not know definitively what causes migraines, but when the attacks happen, those afflicted must pull down the shutters, cover their heads, eyes or ears, and wait for the pounding to pass. Some people see alternating lights and become nauseated; others just get a throbbing head pain.    But while the roots of the disease are mysterious, there’s no dearth of factors that can cause painful headaches. Here are top five triggers for migraines.

1) SENSITIVE BRAIN CELLS

In spite of how common they are, scientists don’t understand what really happens during migraine headaches. But one theory says that the brains of those who suffer migraines are more sensitive than those of other people, according to Frederick Freitag, the director of the headache clinic at the Baylor University Medical Center in Texas. When people get a migraine, brain cells may trigger an electrical wave that spreads across the brain. This wave triggers the release of prostaglandins and serotonin as well as dilated blood vessels — all of which cause a killer headache.

2) IRREGULAR PATTERNS

While some people identify triggers, such as certain cheeses, red wines and hunger, that seem to set off throbbing pain, it’s difficult to figure out because headache frequency is variable, and can by caused by many triggers interacting, says Freitag. So while throbbing head pain may be caused by the glass of wine at dinner, it’s also possible that other factors play a role.

3) HEAD HORMONE

Many women suffer migraines, and that’s no surprise: Hormonal shifts can play a role in triggering migraines. A 2012 study in the “Journal of Headache and Pain” found that half of women who suffered from migraines were likely to experience pounding pain during their periods. But while hormones can be the cause, they can also be a cure: Another study found that the love hormone oxytocin can reduce headache frequency.

4) FLASHING LIGHTS

Almost 85% of migraine sufferers say they are very sensitive to bright/ flashing light during attacks. But another recent study found that trigger isn’t as strong as was thought: When patients were subject to flashing lights, they had an uptick in headache frequency, but not enough to show that the lights always triggered headaches.

5) LIGHTNING ON THE BRAIN

Another type of flashing light can also trigger a pounding head — lightning from a thunderstorm! A 2012 study from the journal “Cephalagia” found that patients were nearly thirty percent more likely to suffer migraines on days where lightning struck near their homes. Although it’s not clear how the lightning strikes could cause throbbing headaches, one possibility is that changes in charge in the air could disrupt the brain’s electrical waves.

When patients start telling me they feel like their muscles are shrinking and tightening to the point that they’re in constant pain, I have immediate concerns about a common disorder that has no cure.  Serving those who live in Dallas, Plano, Addison, Carrollton and the nearby communities I have started to see an increase in patients discussing these symptoms.

Fibromyalgia (muscle fatigue syndrome)  is a mysterious and complex disorder of the central nervous system which produces symptoms of muscle fatigue, stiffness, poor sleep, and restless leg syndrome just to name a few.    Medical experts are not completely sure of the cause, but people with fibromyalgia appear to have abnormalities in the neurotransmitters that transmit nerve impulses.  Research has also found that people involved in traumatic situations, both physical and mental, are prone to having this disorder, especially women and people over 40.

Diagnostic Testing

Because fibromyalgia produces different symptoms in different individuals, I strive to focus on being a good and discerning listener. It is important that my patients have confidence in my abilities to help them with their pain relief along with having proper communication.

When I meet with the patient at the initial consultation, I review their entire health history and perform an overall physical assessment.    Diagnosing and testing can be subjective but the most important test consists of the 18 designated points along the upper and lower back and gluteal regions that will prove to be increasingly tender when proper pressure is applied. In healthy people, the tender points only hurt when enough pressure is put on them to make the base of the thumbnail turn white. In people with fibromyalgia, the tender points will hurt when much less pressure is applied.  To be conclusive, 11 of the 18 points have to be rendered as “painful”.

Chiropractic has traditionally been sustained by semi–qualified ideas like subluxation and intelligence that are not based on real science. Some chiropractors have been criticized for having an anti-immunization viewpoint, in spite of the consensus of health professionals on the benefits of vaccination. This has led to negative impacts on public vaccination and general acceptance of chiropractic. The American Medical Association called chiropractic a cult, and boycotted it until losing a court case. Chiropractic has developed a strong political base and sustained demand for services. It has gained legitimacy and acceptance among physicians and health plans in the U.S. for treating musculoskeletal conditions. The principles of evidence-based medicine have been used for research studies and to generate practice guidelines. Traditional chiropractic still assumes that a vertebral subluxation interferes with the body’s “innate intelligence”, an idea ridiculed by scientific and healthcare communities. Some chiropractors want to separate themselves from the concept of innate intelligence. A researcher wrote in 2011 that chiropractic was at a crossroads, and to progress – it would need to embrace science.

Chiropractic’s early philosophy was based in spiritual inspiration and rationalism. A philosophy helped distinguish chiropractic from medicine, provided it with legal / political defenses against claims of practicing medicine without a license, and allowed chiropractors to set themselves as a legit profession. This philosophy, taught to scores of chiropractors, rejects the reasoning of the scientific method, and relies on deductions from first principles rather than on the materialism of science.

However, most practitioners accept scientific research in chiropractic, and most practitioners are “mixers” who combine materialistic reductionism of science with metaphysics of their predecessors, and with the holistic notion of wellness.

If you have a headache, you’re not the only one. Ninety percent of Americans deal with headaches daily. Some are occasional, some frequent, some are dull and throbbing, and others cause debilitating pain / nausea.  What do you do when you suffer from a killer headache? Do you grin and bear it? Lie down? Take some meds and hope it goes away? There are better choices.

Research shows that spinal manipulation – the basic form of medical care provided by chiropractic doctors – is an effective treatment for tension headaches and headaches that originate in the neck.  A medical report released in 2001 by the Duke University Evidence-Based Practice Center, found that spinal manipulation resulted in practically immediate improvement for those headaches that originate in the neck, and had way fewer side effects and longer-lasting relief of tension-type headaches than commonly prescribed medications.

A 1995 study in the Journal of Manipulative and Physiological Therapeutics found that spinal manipulation is a solid treatment for tension headaches and those who ceased chiropractic treatment after four weeks experienced a sustained therapeutic benefit – versus those patients who received a commonly prescribed medication.
